UDID Card Online Registration Process

UDID card is a Unique ID for Persons with Disabilities. It a single document for the identification and verification of a disabled person to access various benefits. The card contains all necessary details about a disabled person and bypasses the need for carrying multiple documents. The Department of Empowerment of Persons with Disabilities initiated the UDID project to encourage transparency, efficiency, ease of delivering the government benefits to the person with disabilities and also to ensure uniformity.

Procedure to Apply Online UDID Card & Disability Certificate Application Form 2021

Who can apply for a UDID card? All Person with Disability/Disabilities can apply for it. Here are the procedure and guidelines on how you can apply online for fresh UDID Card.

Step 1- Visit the Official Website of UDID Card i.e. swavlambancard.gov.in.

Step 2- On the Homepage, click on apply for disability certificate and UDID.

Step 3- The application Form page will be displayed on the screen.

Step 4- Now enter the required details – Four types of details are required:

  • Personal Details including Address
  • Disability Details
  • Employment Details
  • Identity Details

Step 5- Using PwD logs credentials click on Apply online for Disability Certificate. (Reads instructions and enter the required details).

Step 6- After entering details upload a colour passport photo and other requisite documents. (Identity Proof, Income Proof, SC/ST/OBC proof etc as per requirement)

Step 7- Submit all the data to the CMO Office/Medical Authority for verification.

Step 8- The CMO Office/Medical Authority will verify all your data.

Step 9- After verification, the CMO Office/Medical Authority will assign the concerned specialist(s) for assessment.

Step 10- Specialist Doctor assesses disability of PwD and gives an opinion on disability.

Step 11- Medical Board reviews the case and assigns disability percentage. CMO Office prepares Disability Certificate and generates UDID and Disability Certificate.

Step 12- UDID datasheet goes for UDID Card printing and the UDID card will be delivered by post at the address mentioned during online registration.

UDID Card 2022: Online Application Form

UDID Card 2022: Online Application Form PDF Download – “Unique ID for Persons with Disabilities” project is being implemented with a view of creating a National Database for PwDs, and issuing a Unique Disability Identity Card to each person with disabilities. The project will not only encourage transparency, efficiency and ease of delivering the government benefits to the person with disabilities but also ensure uniformity. The project will also help in streamlining the tracking of the physical and financial progress of beneficiaries at all levels of the hierarchy of implementation – from village level, block level, District level, State level and National level.
