WB Swasthya Sathi Health Scheme Online Registration Process

West Bengal Chief Minister Mamata Banerjee announced that the “Swasthya Sathi’ health scheme of the government will cover the entire population of the state. The decision will be applicable from December 1, 2020 and all people in the state, irrespective of their religious affiliation, caste and creed or profession will be covered under the scheme.

The applicant has to show documents like Aadhar Card, Voter ID and /or Digital Ration Card and fill in an Application Form for this purpose. After submission of the Application Form, the beneficiary gets a SMS with ID-District Code/Serial number. A Unique Registration Number or URN is generated and the Smart Card is delivered within a week. Additions/deletions/corrections are done at District kiosks. Enrolment of applicants will also take place at the State Government organized outreach programmes and camps referred to as “Duare Sarkar” or Government at Your Door-step. Mop-up Camps will also be organised for further enrolments.

Procedure to Apply Online West Bengal Swasthya Sathi Scheme Application Form 2023

Step 1- Visit the nearest government hospital to apply for the scheme. the enrollment will take place in offline mode.

Step 2- Applicant can register through agents at the village medical centres nearby.

Step 3- Agents assist the people in the offline application process.

Step 4- After that, on the application process and applicant must attach the photocopies and originals of all documents mentioned in the scheme guidelines as the enrollment agent will verify the required documents.

Step 5- The enrollment agent can help the people to log in and fill in the registration form on behalf of the candidate.

Step 6- Once the enrollment form is filled and submitted, the site will generate an ID and password for the candidate.

Step 7- Now, the applicant can officially log in on the scheme site with the ID and password.

Step 8- The Swasthya Sathi Smart Card will be generated via the official portal. After logging in, the applicant can acquire this digitized card.

The scheme, officially launched by Banerjee in December 2016 is a basic health cover for secondary and tertiary care up to Rs five lakh per annum per family.

Is OPD treatment covered?

No. OPD treatment is not covered in isolation. However, if it subsequently leads to hospitalization, then it will be covered, as part of pre-hospitalization expenses.

Is transport allowance payable?

Yes. It is limited to Rs. 200/- only per hospitalization and Rs. 2000/- in a policy year. However if the treatment is done  in any Government Hospital through Swasthya Sathi card additional transport allowance is provided and beneficiary will get Rs 400/- to Rs 700/- as Transport allowance.

Who are the current beneficiaries?

Indian citizens who are ordinarily resident in the State of West Bengal are eligible for coverage.

What is meant by cashless service?

Cashless service means that the patient will not have to spend any amount from his pocket for hospitalization and treatment at an empanelled hospital.

What is meant by floater cover?

Floater cover means that the entire family enrolled in the scheme has a maximum and specified amount of Rs 5 lakhs at their disposal for treatment by hospitalization in an empanelled hospital. Any member of the family can be treated, even simultaneously, as long as the total amount is within the annual ceiling of Rs 5 lakhs.

SCHEME FEATURES

    **My wife expired 23rd February 2020. I am alone. No other member in my family. Not getting any Government health care benifit. Not BPL.

    2. My mother can be head of the family, but she gets Defense facilities, So should I have to include her name in the form or simply I can mention my name (Private Job) with my wife (Housewife) and daughters name in the form.

  14. Although it is a good scheme launched by Hon’ble Chief Minister, Miss Mamata Banerjee, I just want a clarification from Govt.
    In the scheme. it is said that this swasthaya sathi smart card will will be alloted to
    entire papulation of West Bengal and in the hand , it is also said that so called smart card holder should be a BPL category.
    I am a senior citizen without a BPL Certificate. So will my family be entitled to get SMART CARD of SWASTHAYASATHI SCHEME?

    But difficult to submit form in our area i.e. Rajpur Sonarpur Municipality, Ward No. 16. I think difficult to submit form in Covid situation such a big que. I appreciate if duare Sarkar any political worker collect ward wise.

  36. I am aged 71 yrs and my wife is also aged 68yrs. My only daughter is 36yrs age and she is Orthopefically Handicapped again she is Thalassemia Patients. Till today 25.01.2021 I am not able to submit my Sasthosathi Fill up Form with documentary papers to our Borro Office.
